Look inside the tank, and you'll find the serial number and possibly the brand...Then look up Plumbing suppliers,and shop around.
ANS 2 - The brand is almost always on the toilet, between seat and tank . The lid will often have 2-3 numbers inside it. Note them all and go to a major plumbing supplier, they will be able to track the number and order a lid.

As far as i know, you cannot buy just the toilet lid. Either you can ask at certain scrap dealers or you will have to buy a new cistern, if only it is a low level. Otherwise if it is a close coupled sytem you will have to replace the entire toilet.
