Angel fish have been known to go into a stage of shock where they lay down on the bottom of a tank and look dead. Provided the water parameters are good for Angelfish (Pterophyllum sp.), if the fish is left to its own devices it will recover and settle down. If however something is wrong with the water parameters the fish will most likely die.
The only instrument that you need when cleaning a fish is a knife. You can clean a fish anywhere as long as you have water. Some places have restrictions on where you can clean a fish.

I suggest that you learn more about your fish in your aquarium. One of your fish might be a bottom sucker which means that they eat all of the dirt on the side or bottom of your tank. If one of your fish are bottom suckers, that's probably the reason why your aquarium is clean. If none of your fish are bottom suckers, than somebody must be secretly cleaning your tank for you.

I would automatically assume that the water is poisoned. Quite possibly something has died and is rotting in the pond. (Bird, Cat, Fish?) Maybe someone has deliberately poisoned it? I have known it to happen. I know it may be very hard to do but a complete clean out and water change could be the only thing you can do to save the fish. Maybe do a very large water change first and see what happens. Good luck with it.
