If the card that has the radio code is missing, you can find a sticker with the code from either the coin holder (the suede covered box in your center console) on the inside of the lid, or underneath it if you remove it, or in the inside of your glove compartment. If it's not in either of those places, you will have to get the code from the Honda dealership.

Try disconnecting the negative battery wire, leave it off for ~5 minutes, then reconnect. Start the engine. The radio should read "CODE". Using the station preset buttons (1 through 6) enter your code. Then turn the radio on. Works for my '93 Prelude; hope this helps you!
