Drill a hole in the the door in the exact same place that the hole is on the driver side door. Remove the inside door panel and put tape on one side of the wrench and place the nut in the wrench and screw the retaining nut on. Drill the hole only thru the outer skin, a cone shaped veri-drill bit works best.
In concept it's easy simply to drill on the opposite side as your existing mirror, but in practice this is something you definitely want to plan and get right the first time! Fortunately, Mid-America Motorworks provides a free template on their website, for this exact issue.
