The 2005 Nissan Altima auto tensioner pulley can be removed by removing the center bolt. You will need to hold the pulley stationary while you loosen the bolt.
remove belts idler is to close to fire wall to come off after you remove nut.jack up car and put stands under.how put a jack under engine . remove top motor mount bolt and jack engine up this will give you the room to change idler pully.

1st remove the pully then you will see the bolts to remove the housing good luck
if ur talking about a serpetine belt u remove the old one by releasing the tension on the belt by put a 1/2 drive ratchet into the tensioner pully and release the tension on the belt and remove it then follow the diagram on how to run the belt and then pull the tensioner and place belt with that pully into the pully drive run and then release and ur new belt is on
first remove the belt by inserting a 3/8" drive in the square hole on the tensiner, then remove the one bolt just below the tensioner and then remove the center bolt on the pully (allen wrench).
With a self adjusting tensioner pulley, there is no adjustment for tension.

It should be a 16mm on the front, to release and put tension on. To remove the tensioner pulley completely, I believe the nut that holds it on from behind is a 15mm.
It more than likely needs new bearings. That is why it is out of line, unless the vehicle has been in an accident. Remove the belt, and remove the tensioner idler pulley and repair it.
Not hard to replace. Take intake tube off. Loosen the belt by rotation the tensioner Remove belt off pully Remove 3 bolts holding it on Take out & replace in reverse order Depending on the alt you may have to swap the alt pully
serpentine belt what i found was that the key way in the crank pully had broke. it then destoyed the inside of the pully and took about 8 thousands of the end of the crank. has anyone else had this kinda promblem?
Lift the hood. Look to the left front of the engine. The alternator is right there. You will see the belt run around its pully, wires going to it, and a tensioner bolt.
