OK, I stumbled through it with the help of an answer to a different question on a 2005 Chevy. The actual project was replacing the heated power door mirror; removing the inside door panel is required because of the mirror's wiring.
1. Lock the door. If you're replacing the mirror, roll down the window. You'll be glad you did later on.
2. Remove the triangular plastic trim molding on the inside of the mirror. There are two insertion clips: one near the top and one to the right. The bottom has a sort of hook that comes away easily once the two clips are out. DO NOT pry on the bottom of this piece! You'll break that hook off and find yourself at your dealer buying REALLY expensive pieces of plastic!
3. Behind the door pull handle there is a hex-head machine screw. Remove it. It's a 7 mm head.
4. On the underside of the door, just about dead center, there's another hex-head screw that holds the storage bin in the panel. Remove it. It's also a 7 mm head. Remove the storage bin.
5. Remove the manual door lock operator trim. Use a small flat screwdriver inserted under the operator panel at the rear; it will pop right out.
6. Behind the door lock operator is another hex-head screw. Remove it. Yep, it's 7 mm too.
7. There is a trim panel insert that surrounds the door opener handle; remove it with a small screwdriver inserted along the rear edge.
8. The door panel is now free to move. Just grab the door pull handle and pull upward; it should move and disengage all the plastic hooks.
Things to watch out for! Once the panel is loose, depending on your truck's equipment, there may be a LOT of electrical cables connecting the panel and the door. Watch especially the courtesy light cable at the rear of the panel, as it is small and relatively fragile. Also, have a care when manipulating the loose panel. The inside has a lot of molded plastic hooks that can catch on wiring or damage your door speaker.
1967 to 1971 will interchange. 1972 will fit but has different inside door panels and hardware.

You will probably have to remove the whole inside door panel in order to remove the handle. The panels are usually held in place with screws or plastic clips. A Haynes Manual might be helpful.
