It is quite a chore. The various Volvo forums have detailed instructions, the quick answer is you have to pull the front fascia off the car to get to the lens assembly. Once there, there are some clips that hold the lens on.

Yes they can. The lens are clipped on and have a gasket between the lens and the reflector.
If you're talking about the headlight, the "lens cover" is not a separate part, you need to replace the whole headlight assembly.
I assume you mean HEADLIGHT LENS.. .. there are 3 bolts on the back, remove, pull out headlight. Remove clips and broken lens. Replace gasket, lens and put on clips FRONT TO BACK so you don't chip the lens. Reinstall with 3 bolts on back.
I think the lens cover has snaps along the top and sides of the lens cover. It also looks as if you would have to loosen the turn signal unit that sits beside each headlight. The headlight lens cover is tucked into a recess along the turn signal unit. It doesn't look as if you would need any tools to remove the old lens covers, but you may need a screwdriver to loosen or remove the turn signal unit next to the lens.

There aren't headlight covers, it all one unit. One of my lens was cracked and I had to replace the whole unit.
Where the lens meets the housing there is a seal, just replace it.
You don't. Instead, you replace the entire assembly.
