I find it's easier for me (maybe not anyone else) to just go ahead and lower the front facia. there is a screw in the upper part of the wheel well that you need to remove, then remove two screws along the top rear of the bumper cover, then remove the 4 plactic holding pins along the top of the grille/bumper cover. Pull it out and unbolt the head lights. But you can just remove parts of the air intake and go that route. they are just hard to get to. I have the HID version so I just go ahead and take off the front cover as not to damage the bulbs. Dunno if that helps or not.
The Low Beam AND High Beam are in the same bulb. The procedure to change it will be in the owners manual.

If only the low-beam is out, you will need to replace the low-beam bulb. It is probably not necessary to replace the entire headlight assembly.
The panel containing the Headlamp fuses for the 2003 Altima is in the right front corner of the engine compartment. It is in front of the Alternator. This panel is vertical and each lamp has its own fuse, Low Beam-Front Left, Low Beam-Front Right, etc. There is a clip holding the panel in place.

You Have a bad headlight, it's common when a headlight burns out for only the high beam to work, solution change your headlight.
A sealed beam headlight is just that. The unit is sealed and you replace the entire light. A composite is made of pieces that's why the name. To replace a composite you put a new blub in the back.
Carry it to dealer who will replace on low beam headlight for $15 +/- dollars in about 5 minutes.
Passenger side or driver side? BClear. The low beam is built into the same bulb as the high beam. Just pull the bulb out from behind the housing under the hood and replace it. If the low beam doesnt work you have to replace the whole bulb. They're not very expensive.
The headlight has two filaments. One for the high beam and one for the low beam. The most likely reason the high beam is out is because the high beam filament is broken. Replace the bulb and it should fix it.
