You are going to have to remove the brake drum(s) to to this. Once off find a nut of matching thread to the stud or use an OLD lug nut. Place the drum on a flat surface with the lug studs pointing up and pound the offending unit back through the drum. If no threads are left then get a piece of rolled steel stock or a thick drift pin (about the thickness of the stud) and pound away until it lets go. Lacking the personal wherewithall to do this yourself, any machine shop and even some parts houses can assist you with this one.

It doesn't have struts. It has coil-over shocks.
The starter on a 1998 Toyota 4runner is held located on the transmission housing. The starter is held on by 2 bolts which mount it directly it the engine. Remove the two bolts to replace the starter.
The wheel studs are replaced by first removing the tire from the brake. Remove the disk brake pads and caliper if it is the front and remove the brake drum if it is the back. Once you have the brake disk or drum off, simply pound the studs with a heavy hammer and they will pop out the back. if you plan on reusing the studs put the nut on the threads so it is flush with the end and then use hammer as not to ruin threads. Spray WD-40 or heat the brake disk/drum(not the studs) if they are stubborn or rusty.
