How do you respond to someone who is apologizing and you are still upset?

The standard tactic would be to tell the person you are too upset to discuss it at that time, but also tell him or her that you are willing to revisit the issue at a later time.
A rougher tactic (depending on the intensity of the offense), would be to say "I hear your apology, but I don't know how I will feel about you later. Only time will tell".
EDIT: My friend posted the above, which is fine, if your an adult, but reading your question I think there is a fair chance you are under 20, maybe 15ish, and therefore the reply you get from using the above advice could be quite diffrent than expected. So incase you are around the 15 year old age, I would advice that you smile and nod through his apology and look to your close friends for advice, for they will hopefully give you a true 3 person view on what is happening then you will see what it looks like to other people which is usually more accurite.

Honestly, I'd say it isn't valid because if someone says, "why are you still mad?-" Or something and you respond with "BeCaUsE" that's not quite a valid answer since you didn't explain why you're still upset.

If you mean that someone you don't know, labelled you as angry, then yeah of course it's okay to be upset by it. They have no right to label you as that if they don't know you. Even if you are angry, or come across as angry they still have no right unless they know you well enough. Even then it could still be upsetting.
