First, write a letter stating the facts -- be clear on dates and details.

These types of purchases were the only purchases I made, typically from small neighborhood stores and from a retail pharmacy nearby my employment. When I left that employment, the credit card was (give what happened to the card, such as: the card was left with my employer and I had no further access to the card). I never made any recurring or revolving charge on the credit card.


On May 2, 2013, I discovered this card is on my credit report, showing a recurring unpaid monthly charge of $32.00 from (give the business name or company for the product or service). There is no indication when this charge began, but it continues as of the date of this letter. I did not make or authorize these charges. I do not know who made or authorized these charges.


This letter is to request that:

1. The card be closed or restricted for any future charges;

2. The card be removed from my name;

3. The creditor, bank, or other authority investigate the recurring charge and notify me of the results of their investigation;

4. The store or business making this charge stop charging against my name;

5. That the store or business making this charge provide details of who initiated this charge, AND issue a correction to all 3 Credit Reporting agencies that removes my name from any charges beginning May 2012, or now or in the future and for the balance due.

(List any paper documents that prove what you are saying. Examples: Last pay stub from that nanny employer; your address at that time; any credit card statement you kept or any proof you have of purchases you made; etc.)

Second, Xerox copy your letter. Mail it by Registered Mail with Return Receipt Requested (costs money but well-worth it). Send one to:

1. Each of the 3 credit reporting agencies

2. The business making the recurring charge. Include a note demanding they stop the recurring charge against your name and do an investigation.

3. To the actual credit card - Visa, MasterCard, etc.

4. The bank that issued the card.

5. Your ex-employer.


Third, you might need help. Try Consumer Credit Counseling agency, or an attorney, to help you clean up the issue and get it off your credit report.
