Check the timing belt to see its off.. If the timing is correct them most likely you have a failed camshaft sensor..
Yes you can just replace the headlight bulbs. First you have to take out the cover by the battery. Then unscrew the cover by the radiator and simply lift it up to take it off. The light bulbs are visible at this time.
If you do not have it written down in the owner's manual, you need to contact a LEXUS DEALER. You will more than likely need the serial# off the radio. That will require removing the radio. Check with Lexus to b sure.
This is a common issue with Lexus, mainly with the older LS400's. Basically, the instrument cluster needs to be replaced, this can only be done through Lexus. A new cluster to be installed including labor will run anywhere from $750 - $1000. If you can find an instrument cluster at a salvage yard, Lexus will send it off to match the mileage from the original cluster, then install it for 2 - 3 hours of labor cost.

The timing is computer controlled, there is no adjustments. I know this is a little late, but timing could be off because the timing belt is not properly aligned. If the timing belt is installed incorrectly the timing will never be right.

The 4-speed automatic transmission in my 1990 Lexus LS400 developed problems at ~220K miles, and the 5-speed automatic in my 1999 Lexus LS400 got sick at ~160K miles (although both were still driveable).Each cost around $3000 to rebuild (at a transmission shop... Lexus service doesn't rebuild anything).Same driver, same driving conditions (in Houston, mostly stop-and-go), same maintenance schedule (fluid change approx every 50K).So I suppose it's a roll of the dice what you should expect.BTW: I still have both cars. Of the 30+ (mostly American-made) cars I've owned, these are by far the best-built (although I've never had a Mercedes). I plan to drive them 'till the paint wears off!
