A business letter will not prove that someone lives with you. Contact the party that requires the proof and ask what documentation they will accept as proof. If you only have the address of the party (no phone number or email address), use the standard format business letter (below) to request that information.
Once you know what documentation will be accepted as proof, attach that documentation to another letter, using the same standard business form and list the documents at the bottom of the letter at 'Enclosures'. Sample business letter format:
Letterhead or sender's address.
Date the letter is written or sent.
Inside address, the name (optional), title (optional), organization name and address.
Reference line (optional)
Salutation (greeting): 'Dear Mr. Jones', 'Dear Sir/Madam' (if name/s unknown), or title of person if known, 'Dear Prof. Jones'. Always use a formal greeting for a business letter.
Body of letter:
first paragraph, state why you are writing the letter.
second paragraph, state the information supporting your request. Use one paragraph only unless there is a lot of related information, in which case use bullets if possible.
third paragraph, tell the recipient what you want them to do, giving them any information necessary for them to follow through (your phone number, your email address, etc.). Be sure to state any deadline date, if applicable. Always thank them for their time and/or effort.
Complimentary closing such as 'Sincerely' or 'Best Regards', etc.
Signature of sender.
Typed or printed name of sender, with title if applicable.
Enclosures, if applicable, list anything included with the letter.
When signing a letter on behalf of someone, write the abbreviation p.p. in front of your signature. The absent persons name is then written on the next line down.
