It is driven by a gear off the backside of the distributor housing, with fluid remotely fed by a tank located on the right fender. Low pressure fluid (ATF recommended) runs from the reservoir to the pump, high pressure from the pump to the rack, then the spent fluid now low pressure again to the cooler located on the left side of the radiator, and then up from the bottom of the cooler to the reservoir. Inside of the pump is pretty much unserviceable, and is built similarly to a A/C compressor, with a series of plates and holes that move fluid at alternating rates from front to back to build pressure.

Begin by removing the power steering pump belt from your 1991 Cadillac power steering pump pulley. Remove the power steering pump pulley retaining bolt. The retaining bolt will be in the center of the pulley.

Remove the power steering pump hose. Remove the power steering pump belt. Remove the power steering pump retaining bolts. The power steering pump will come off.
Secure the power steering pump with the retaining bolts. Attach the power steering hose to the power steering pump. Put the belt on to the power steering pump. Fill the power steering system with fluid.
Remove the belt for am your power steering pump. Remove the power steering pump hose. Remove the power steering pump retaining bolts. Reverse the process to install your new power steering pump.
Drain the fluid from the power steering pump. Remove the power steering hose. Remove the wiring harness from the power steering pump. Remove the power steering pump retaining bolts. Reverse the process to install the new power steering pump.
