I just fixed my horn by remounting the screws holding the horn assembly to the front grille area. So it could be that the ground is through that screw. The horn will not work with that screw removed.
In our 2002 Toyota Sienna, it's behind the felt lined drawer on the bottom left of the steering column.

The fuse box on a 2004 Toyota Sienna is located between the hood release and the steering column. It can readily be accessed by the driver from the floor board.
Press the PWR DOOR OFF button to the left of the steering column.
It is located underneath the steering column. You have to lay on your back in order to look up at it with your head near the pedals.
The flasher on a 99 Toyota Sienna should be beside the instrument panel fuses under the steering column. You will probably have to remove the driver's side knee bolster to get at it, which is kind of a pain.
There is a push button that can be pressed up or down (it is rectangular in shape) located below the air vent to the left of the steering column.
