Any belt will wear out prematurely if the belt turns a pulley with bad bearings. A new belt should last 50,000 miles but if it wears out before this time you must seek a fault with another of the pulleys bearings. Should be checked every 15,000 miles. Ours last 106,500 but not a belt you want to break. The vehicle does not run at that point. I would say every two years depending on the type of driving the vehicle does.
Go to www.autozone.com and look under specifications, belt routing. On a 1989 Dodge Grand Caravan, the diagram is on the side of the engine where the S-belt is located.
