In the US, there is no requirement for public bathrooms to have showers, handicap or otherwise. In newly renovated or new establishments, if there are showers available there must be at least one that is designed to be accessible for disabled individuals.
The public bathrooms were locked
Bathrooms are generally public.
There used to be public bathrooms in the subways, but they were closed, in the early 1980s, I think. The city decided to close the bathrooms because so many people were doing drugs and prostituting themselves in them. Now you usually have to find a Starbucks or some other restaurant that will let you use the bathroom. There are a few public bathrooms in the city: there's one at Bryant Park, on West 42nd Street between 5th and 6th Avenues, for example. However, it closes when the park closes. Most police stations will also let you use their bathrooms. There's also a public bathroom at Grand Central Station, and at Penn Station. I have included a link to a site called "Clean Bathrooms in New York City," which lists some more places where public bathrooms can be found.

There are many many more bathrooms. The White House has many more offices in it than bedrooms. There are dozens of bathrooms for the many offices, as well as bathrooms for the bedrooms.
