All sugars that are common in any diet are natural. Although fructose and glucose can be bought, the huge bulk of sugar that is added to foods, either before or after cooking/preparation, is sucrose. Medicine does not differentiate between refined and unrefined sugars - using sucrose with small amounts of caramel does not change its nutritional qualities. (Despite what the health food industry might imply). There is no recommended limit, just a recommendation to minimise intake.
