Not very often and certainly nowhere near as often as the popular media would lead one to believe. Because of their usual high profile they are invstigated fully and for the most part carefully. Once law enforcement is through with the case it is sent to the prosecutor's office who also looks it over with a fine tooth comb. Have mistakes been made? Yes, they have. Will mistakes be made in the future? Probably. Any human endeavor is fraught with possible error but for the most part they are among the most thoroughly and carefully investigated of offenses.

Why someone was not invited to a shower?

When someone is not invited to a shower it does not mean that person is not liked, but it is probably the mistake of the hostess giving the shower. Sometimes the hostess has to rely on the boyfriend/husband, etc., to give a list of the woman's (who the shower is for) names if her friends and one or two often are missed.
