This is titled as a "reverse flag patch" and signifies or displays the flag as moving forward! Many naturally assume that the flag should be worn in the position closest to the heart. It depends on the agency's or department's unifiorm regulations. Some wear a patch on their shoulder - some on the chest - still others wear the flag as a pin on their uniform or attached to their nametag - some others do not wear it at all. .
The stars are always forward on a patch as if it were being held in a procession. The reason for this is it is symbolism for us always progressing and never retreating.
