If your 1990 Nissan Sentra will not shift any higher than 1st gear and reverse is fine is it a problem with the modulator?

It is probably the governor gear. It is accessible from outside the trans. Put a new one in, drop the trans pan, drain all fluid and clean out the filter and refill.
