Im not sure if it can cause permenant damage on your internal organs but it is safe as long as you don't abuse it. Taking over the recommened does for your weight (around 2-5 grams) can but stress on your liver and kidneys due to the fact it has to excrete the excess creatine out of your body. The creatine is excreted as a waste product called creatinine which can but a lot of stress on your kidneys and liver. Over lots of time this stress can build up and cause serious damage.

Yes it is bad to take caffeine with creatine. Both caffeine and creatine dehydrate you so you will get really dehydrated. Even when on creatine alone you should drink a lot of water because creatine needs a lot of water to work and you will get dehydrated if you don't drink water when on it. Also caffeine will reduce the effectiveness of creatine.
