Generally, yes. Sparks mean uncontrolled current flowing in places it shouldn't. Often, sparks are generated when something has shorted out. Short circuits generate a lot of unwanted current and a lot of unwanted heat that can melt components of the circuit. So if you're seeing sparks, it's probably already too late and you've fried something.

The only other sparks you may encounter are across switch contacts, particularly with an inductive load such as a motor. Inductive loads will continue pushing current through a circuit, even after it has been opened. This back-current can generate a voltage large enough to overcome the breakdown voltage of air and create an arc. This damages switch contacts. Arcing powerful enough can melt the contacts and/or simply override the switch. So, arcing is generally a bad sign and should be avoided. This is often easily fixed in DC circuits by placing a reverse-biased diode across the inductor or inductive load. Power factor correction will fix this in an AC circuit.

If you have power to a circuit (you can tell with a multi meter), and it does not do what it is supposed to it is bad. Make sure the fuse is good. If fuses keep blowing then there is too much current, which is usually indicative of a short. If you see smoke, there's a good chance you're circuit is bad. An electronics book will tell you what to look for if you are trying to troubleshoot a circuit board. Many times the board will need to be replaced as individual components are too small and impossible to get at to change.

Impossible to give a specific answer without more information. Is it a BJT or a FET? Most general answer is you first have to understand the circuit using the transistor and what it's supposed to do. Then, 1. Check that the power supply voltages are correct. 2. Check that the DC biases are correct. 3. Check that the input signal is correct. 4. Check the output. But you have to know in advance what to expect.
