Technically it's possible, but it needs a lot of knowledge of concrete, and of where exactly the support for the floor is. We are not talking about a small hole, for an average circular staircase, the hole would have to be at least 6 feet diameter. The concrete is likely to be 'stressed', in other words containing steel cables which would make cutting it very difficult. That's a big chunk out of a floor. It may even need to be supported on the edge of this hole in at least 2-3 places.
