Sounds to me as if it was put in some kind of a tool that compressed the coin slightly. There are things called "upsetting machines" that are used to compress metal disks for use in medals, rings, pendants, etc. If someone put a coin through one of those it could cause the effects you see.

Unfortunately, as you might guess, that makes it a damaged coin worth face value only.
