Only one speed would most likely be the resistor or switch. The loud noises I cannot explain unless the motor itself is going bad or the mounting brackets have broken and its vibrating or further still.. somthing has gotten into teh blower motor.. leaf litter, a mouse, etc. ( scope it if you can). on a scope it should look like a SINE-WAVE ( fluid up and down motion kind of like a real wave) if however, you get a scattered, staggering line form, the motor is for sure bad.
One fan speed tells me its simply mechanical, the switch and or ressitor are not diffucult to change on most makes and models.... in hind site.. if you had lost the HIGH fan speed ONLY i would suspect the motor, ignore some of what i said :P
The blower motor fuse protects the blower motor electrical circuit. The blower motor relay operates only the high speed function of the blower motor. The blower motor speed resistor operates the lower speed functions of the blower motor.

1) Check the blower motor, make sure it is not making any weird noises.2) Check the blower motor resistor, its located next to the motor. It is what controls the lower speeds of the motor. You can try to find one at the junkyard.
