is it an automatic transmission? if not does it have a engine start button? in both cases mentioned the vehicle will have a solenoid in the gear shift lever or gearbox that prevents the vehicle being started in gear!
Prevents the vehicle from starting in other than Park or Neutral
On a single phase supply the neutral is one of the current carrying wires, the other one being the live wire. The neutral is earthed at the tansformer and sometimes at other places as well depending on the system in use. On a 3-phase 4-wire system the neutral carries current only when the load is unbalanced, in other words when there are unequal currents in the three live wires.

The neutral safety switch prevents you from cranking the car in any other gear than park or neutral. Sometimes this will fail and you will have to manualy flip the switch. also there is a possibility that the car will only start in park and never start if it is in Netural.

I don't think it's possible to generalize. Sometimes it's one, sometimes the other.
i would say you have an injector problem. usually a cloged one can cause that.
