have you tuned it up? plugs, wires, cap + rotor
if you have standard injectors .you use an ohmmeter. to test for the ohm resistance
rough idle,execsive smoke from exhaust ,low power could all be symptoms of bad injectors . also check the delivery pump pressure it could also cause the same symptoms
dirty injectors,water in fuel,exhaust leak, timing belt,universal joint.Check them all.
Check the air filter, fuel injectors, EGR valve and temperature sensor.

If you have not already replaced your spark plugs then do so, you might also want to check the coils while your replacing plugs if it still runs rough with loss of power try sea foam to clean your crank shaft and your injectors and all the fueling system.
Well a lot,first be sure the wires are in good shape, replaced the plugs,do a tune up.
one of your problems could be in the injectors they maybe putting to much fuel.
I have a 2002 F-150 with a 5.4. The truck idles rough and shakes going between 40-50 mph. I read the codes and it has a cylinder misfire in 8, and fuel is too lean in both banks. I have replaced all 8 injectors a few months ago, along with exhaust manifold, and manifold gaskets. In the past year I have replaced all 8 spark plugs and boots, they are gaped properly. I am thinking it could be either the coil pack in cylinder 8, or a oxygen sensor, or perhaps both. Any assistance would be greatly appreciated.
Try replacing the injectors.
