I was just seen by a Gynecologist for the same problem. I am 29 years old and have lived with endometriosis since a tennager. I always have painful periods and pass a lot of blood clots during my cycle. Duiring this period I passed a piece of tissue the same as you described. I was told that it was a piece of the lining of my uterus. Sometime it thickens and the body expels it.
