Means black smaoke and a failed catatlitic converter
The pressure regulator is defective or an injector is stuck open.
Bad fuel injector, or debris stuck in the injector not allowing it to close.
Constant ground signal from the ECM, or injector could be faulty and stuck open.
Fuel leak, or Fuel Injector stuck partially open.
Get a fuel pressure test gauge making sure you get one that can handle your pressure. Hook it all up. Run engine. Then as soon as you turn off engine, crimp off fuel return line to gastank. If pressure drops you've got a stuck open injector or other leaks.

The injector could be bad if it is not pulsing and just stuck open. Also check the plug and wires for that cylinder bc of the fouling from the fuel.
You have a fuel injector for each cylinder attached to fuel rail on top of engine. These simply open/close allowing fuel in.
If its BLACK smoke it's unburned fuel. If it has carb, the carb float is too high or sunk. If it has fuel injection it's a stuck (open) fuel injector.
You'll need to replace the stuck injector.
This means the engine is running extremely rich. This can be causes by a leaking fuel pressure regulator or a stuck open injector.- TechJK ------------
