Difficulty shifting, can't reach first gear and grinds to reverse, have to stomp on the brakes to downshift, basically any sort of unusual difficulty with shifting, car jumps forward when clutch is engaged or moves while on a flat surface and clutch is engaged.

The clutch should last approx 150,000 miles. There is no need to replace this if it isn't abused
Keep It Simple Stupid. Take it to a mechanic and replace clutch cause any clutch problem means you need to replace it!
It's usually a good rule of thumb that if you replace the trans, you replace the clutch.

You may need to adjust your clutch or replace your clutch.
Depends on what symptoms you're getting. Is it running with vibration? Or is it so badly scored that the clutch won't work? It's most unlikely that you'll need to replace a flywheel. They don't really go wrong.
