In the 1992 Lexus ES300 there is only one cooling fan. The cooling fan is operated hydraulically by fluid from the power steering reservoir. If there is a leak in the fluid pipe the fan can stop working and the car will over heat in traffic, otherwise it is fine. In any case the fan MUST always run when the A/C is switched on. Check for leaks in the rubber hose leading to the cooling fan from the hydraulic fluid reservoir.

Before you can fix it, you need to find where the problem lies. Most common causes of overheating are: thermostat stuck in closed position; faulty radiator fan motor or relay. Other causes are faulty water pump; clogged radiator or hoses; loose belt.
No heat coming from the heater and the car overheating is usually the thermostat stuck closed. The thermostat is supposed to open up at a certain temperature allowing the engine to warm up quickly.

There are two cooling fans in the front of your car. Check to see if they are both coming on when it is overheating. If only one is coming on it either has a bad fan motor,bad sensor that tells it when to come on or maybe only a bad connection at the sensor. You may need a mechanic to check it out unless you know about how electrical systems work relating to cooling fans
Your temp gage may be malfunctioning or you may have a head gasket leak that is making excess pressure build up in the cooling system. Have the coolant temp sensor tested for accuracy. This would also prevent the cooling fan from coming on and cause the overheating condition.
If you mean engine coolant is getting into the car, the heater core is leaking coolant causing the coolant level to drop and that causes the engine to over heat.
