maybe the EGR valve or the cold start injector, with out knowing all the symptoms its hard to tell. Both of these can effect how smoothly the engine runs. They are right next to each other on the passenger side of the intake manifold. you should be able to temporarily remove each and test it out, just block off the holes temporarily.

Try this (might be your issue): Remove the throttle body and idle screw. Flush with Sea Foam Deep Creep spray (found at auto parts store). Brush inside if you can. If you remove your plenum, clean as much as you can inside and out, behind the sensors, cold start injectors, etc. Reattach all parts and start car. This was a fix for my idle problem on my truck. To much carbon build up.
