Worn out rocker nut, stud pulling out, or bad cam.
Each rocker arm has a bushing beneath the rocker arm screw. When the bushing is worn out, the rocker arm will continue to loosen up.
to adjust the rocker arms you have to either tighten or loosen the top bolt on the rocker arm/arms that need to be adjusted. there is a specified torque for the rocker arms that you should follow to help you adjust the them

Runing on a colapsed lifter for to long usualy.
Pull the valve covers off. loosen the rocker arms with out taking them all the way off. then you can turn the rocker arms to pull the push rods out. replace push rods, and tighten rocker arms back down and put valve covers back on

You must have a modified camshaft. The lift on the cam is to high of a lift and the rocker arms are binding on the studs. You need to use a long slot rocker arms to stop that. You also need to check the rocker are studs and make sure they are not cut in half.
well the lifters shouldn't be too tight if they are the correct lifters for your vehicle. if your having problems with your valves opening its probably because your rocker arms are torqued beyond specifications. buy a manual and loosen your rocker arms torquing to specs.
