As atoms, carbon, oxygen and nitrogen are formed from hydrogen and helium via nuclear fusion inside stars. Hydrogen itself is formed from further subatomic particles. As elements, each are formed when atoms of that element collide and form covalent bonds (apart from carbon, which requires large amounts of pressure and heat).
