First you must get your EMT, which can be done at a community college, and takes about a year. Then you can apply for a medic program, taking another 1-1 1/2 year, when completed you will have your paramedic.
To become a parimedic you will need an associates degree. I had to go through a class to become an emergency medical technician, and then pass a state board examination. I then had to take a paramedic class and pass another state board examination. This was 12 years ago, not sure if the requirements have changed.
You need to be a basic EMT before you can become a paramedic. Once you are a basic EMT you can apply at most ambulance companies to get experience. Once you have experience as a basic EMT you can then attend a paramedic course.
The best way to find out about educational opportunities in your area is to go to your local community college and talk with an advisor to see what programs are available around you. The education required to become a Paramedic varies from state to state within the US. There are also at least three route of education available in the US. The quickest route is sometimes called the "zero to hero" where a student enters a program with no EMS training and graduates with the minimal education requirement necessary to take their state paramedic entrance exam. This route is available as an Associate or Bachelor degree program, or as a certification program. Some students work towards becoming a medic a little slower and opt out of the "zero to hero"taking shorter classes to be certified as an EMT-Basic then an EMT-Intermediate before charging into the advanced EMT-Paramedic classes. There are also full-time and part-time classes available to fit the schedule of students. The quickest medic program I am aware of is a nine month full-time certification program than is very intensive and does not have a high pass rate. Most certification programs gradually progress though the EMT levels and takes 18- 24 months. The degree programs are now recommended by many employers. The Associate in Applied Science Degree with Emergency Medical Science concentration takes at least two years full-time. A Bachelor in EMS takes at least four years full-time.

No you do not. There is no eye test to become a paramedic. However you will be treating patients and performing skills where you will need visual clarity. As long as you have proper glasses or contacts that is all that matters.
