A clutch is a disc usually made of metal, with a hub, spokes, and sometimes springs. Some clutches are cone shaped.

Clutch disc? If this is what you are asking, it looks like a plate with an abrasive surface on both sides. In the center is a "splined" hole. Hope this helps
It's going to look like your power brake reservoir. It is the reservoir for your clutch slave cylinder.
Well, not quite sure what it looks like. I have never needed to use one. You just un- bolt the old clutch, and bolt on the new one.
You have to look at the wear on the clutch and look for cracks. If the clutch does go out, the shifter will still move everywhere, but the transmission will not shift.

the plates are round with a cork like coating on the outer surface about 3/4" wide
It is inside of the transmission bellhousing, You can't see it until you remove the transmission.
sounds like your slave cylinder is bad or your clutch, find your clutch fluid reservoir and see if its full ,if not look for a leak if there is a leak most likely that's the problem and needs replacing.

Sounds like clutch slip. Either the clutch springs need adjusting or the clutch plates are worn out.
