Your question is hard to answer because you don't mention what day you are in your cycle. The spotting could be the start of your period or caused by something else. The info in your question is too vague to give you a really good answer.
Stress or weight loss are two possibilities. If you have lost a lot of weight recently it could distrupt your cycle.

Spotting is when you will randomly have a little blood. Not a full blown flow.
A blood test would have been negative 3 days before your missed period but an hpt would ususlly be positive by now. You have to use the first urine of the day when it is good and strong.

you only count days with blood spotting, the brown spotting may occur when you change your diet or over exercise your self. i asked my doctor about the 14th day brown spotting and she said its nothing to be worried about, i guess that should apply to your case.
Not likely, but possible. The most accurate way to find out is a serum pregnancy test given by your doctor. If your periods are always around the same time, I would consider this option.
It's the first day of spotting/flow. I had light spotting this month & thought it was implantation bleeding. But it ended up being a very light period (and all of the PG tests were negative). If you think it might be implantation bleeding, take a HPT 6 or so days after it's done. It takes 4 full days for the embryo to implant, then another few days for the HCG to be detected in urine.
