The reactor tripped and the automatic shutdown systems engaged. The operators misinterpreted the status of a pressure relief valve, along with water level instrumentation and they stopped a pump because they thought there was too much water. By the time they realized the error, part of the core had become uncovered from loss of water and it overheated to the point where it was damaged.
