If you can prove that you ARE are employed - and receive a wage - you should be under the auspices and protection of the Workman's Compensation Act for any on-the-job injury. The matter of no W-2 is a tax matter and will have to be addressed seperately.

What happens if you get hurt at your job as a bartender and you receive a shift pay but you never receive a W-2?

You would be responsible for your own medical bills. You are probably being treated as a self employed taxpayer (contract worker). You will have to deal with the payer of your services about this matter.
