As with any field, you should make sure you have the mental and physical capacity required to handle that job in the most proficient, efficient, and productive way. I'm sure you can find out what the physical requirements are. Then test your self (with counsel of a physician if needed) based on those requirements. This is not a difficult thing to do. Within a ROTC program, you are not going to be jumping out of helicopters, or repelling from a 400 foot drop at night. You may be underestimating your ability. However, there are many individuals who do have weight issues, or physical handicaps that might be a threat to achieving this type of program. Still, you will never know unless you ask. So ask, unless you want to spend the rest of your life wondering if you could have done this (I strongly recommend against this).
Yes, you can, however, you would have to be accepted into that program and complete all the requirements for physical therapy.

A physical therapist is not a physician, and does not go to medical school. Therefore, to become a surgeon, you must complete medical school which is a four year program of study, and in addition, three or more years to complete the internship and residency requirements.
The number of credits does not determine equality to any specific program of study. Each program will have specific requirements. Once you are accepted to that program, successfully complete the requirements of that program, and the registrar indicates you have completed the program and have graduated, then you have that degree.
