The basic idea is that the Cold War continued until eventually it turned into a nuclear war. Some people in America survived for hundreds of years in underground "vaults", and you play one of these survivors, eventually emerging after hundreds of years underground. You find yourself in a post-apocalyptic wasteland, where there's roving bandits, mutated animals, slavers, monsters, and people just trying to get by. That's the general setting for the game.
Fallout is a game set in an alternate version of Earth, upon which the transistor was never invented, and nuclear power was fully embraced, leading to a golden Atomic Age in the 1950's. Then however fuel shortages in the 1970's cause worldwide strife. Europe invades the Middle East, the US annexes Canada, the Chinese invade Alaska, and it all culminates in an all-out global nuclear war in 2077 that brings an end to modern civilisation.
People survived though, either in towns and villages too isolated to be affected by the bombs, or in giant underground habitations called Vaults, designed to house a population until the radioactivity outside has droppd to safe levels.
In Fallout, your character is a resident of one of these Vaults, Vault 13. The Vault's water purification chip has malfunctioned and the water supplies are dwindling. So you've been chosen to find another vault, and find a spare. The post-war world is arid and irradiated, full of 'tribals' (primitive settlements), raiders, mutants and small pockets of civilisation.
well basically, its 80 years after fallout 1 (in which your searching for a water chip for Vault 13) you need to find a G.E.C.K for your village but its nowhere near as good as FO3 mainly cuz its in like birds eye view or something. (I own Fallout 1, 2, and Tactics)

You wake up in goodsrpings to find you've been shot in the head. You learn you were a courier delivering a package, and the whole game (apart from miscellaneous quests) is about you finding the package and benny, the guy who shot you. And then from there you can choose to side with mr. house and run new vegas, the ncr (best karma) or The legion (evil). You can also take companions like rex a cyberdog you can get at the kings place in freeside and veronica at 188 trading post... there are a lot, but those are my favourite.

Chowder138's answer: the answer above is a piece of crap. from what I've heard, the game takes place in an alternate world. following WWII, the US becomes 13 states to improve the economy. this somehow leads to a shortage of petroleum causing a terrible war. skip to 2077. The Great War erupts as China takes over Alaska. during the war, possible the entire country is almost destroyed, turning it into a barren wasteland. that Vaults (such as Vault 101) were created for the safe-being of the surviving humans so as to protect them from the huge pockets of radiation scattered across the wasteland. hope this answer helped.
The plot is roughly that your character, the Courier is carrying a special package, The ******** ****(read: game breaking MacGuffin) that the head of New Vegas, Mr. House 'ordered' to be delivered. Your package was intercepted and taken from Benny, the protagonist of the game, and the first main quest you receive is to track him down, and later you learn more about the package, and it's recipient.
The setting is based on Las Vegas, and the areas around it, but with a post-apocalyptic twist that Fallout is known for. Come create your destiny finding your attacker, while you practice your skills, sneaking, guns, repairing, and find perks, old and new such as Jury Rigging, The Professional, and more.
The game is flavored nicely with different currencies, and many other different possible variables.
Occasionally Fallout: New Vegas will freeze, but not as frequently as Fallout 3.
