There is no particular app to make a person look skinny, it takes a lot of manual photoshop work. The way an artist would normally go about it is by selecting an area of the body with the Polygonal Marquee Tool (or creating a Quick Mask), then using the Liquify filter, and very gradually nudging parts of the body bit-by-bit inwards with a low strength brush(the size of the brush can be adjusted up or down as needed using the [ ]keys). The artist will also often lighten shadows that were created by bulges in the flesh (where less light hits,creating shadow) by cloning nearby areas of skin that are lighter or very gradually/subtley using the Dodge tool on them with a low strength brush until the desired effect is achieved. The artist will then use the Clone Tool and/or Content Aware Fill on the surrounding foreground or background areas, to fill in the blank area left over in the image from where the body used to be.
