A Pressure Plate
is a type of switch that can be placed onto a surface of a block. If connected by wire to an object (or placed adjacent to ablock that can be powered), they will provide power to that object or block when stepped on by either a player or a mob (as well as other items; see below), and will stop when stepped off. Pressure plates are one of the seven types of switches currently in the game.
In automotive terms, it is part of the clutch system of a standard shift vehicle.
The clutch will have to be replaced. The pressure plate could possibly be resurfaced but usually it is as cost effective to replace the pressure plate.

The pressure plate is what the clutch presses against to drive the transmission. The plate is a metal round disk that needs to be smooth for a good connection to the clutch.
It's not connected to the cover. The pressure plate is bolted to the flywheel with the clutch disc sandwiched in between.
Applies pressure between the clutch disc to the flywheel.
