The nut is not holding the rotor on. It is holding the CV shaft into the bearing hub assembly. The rotor is only held on by the caliper and lug nuts when you have the wheel on. Remove the wheel and the caliper. The caliper bolts are probably 3/8" hex bolts. If the brakes have never been changed, there may be a small metal retainer around one of the lug studs, but being a 96 model they have probably been changed and the retainer is no longer there. It isn't necessary to put another retainer on, that is only to hold the rotor in place as it goes down the assembly line in the factory.

Your Lug Nuts should be 19 mm and there should be 5 of them. That large nut in the middle "the center hub" leave it alone. It has nothing to do with front tire other then holding the axles and rotor together.
Actually the nut does not hold the rotor on. Your brake caliper does that. The nut holds the front hub on. correct and the torque setting for the front hub nut is ..284 ft pounds

No, the rotor will come off with out the hub. After the brake caliper is off the only thing holding the rotor is rust. Hit between the studs with a hammer and it will come off.
To change front rotors on a xterra u have to remove the brake caliper and brake guard off the rotor and then remove the gold cap off the hub and unscrew the two screws holding the wheel bearing in then unscew the holding plate that holds the bearing remember which way the holding plate goes when taking off... then remove the hub and the rotor comes with it ... then the screws going around the hub on the out side remove theses and the rotor comes off .. but i also suggest repacking the wheel bearings and replacing brake pads pads with the new rotors ...
