In my opinion, all of them. They are out to make money and buy the cheapest ingredients they can. Reclaimed meat, where the animals used carcass is pushed against a metal grill using a hydraulic ram. Tissue is squeezed through the grill which is all that is left on the carcass after they have removed the main meat. It is then collected and formed into shapes before cooking and selling as burgers etc. They also exploit their employees by paying low wages and treating them badly. They also have little training particularly in the area of hygene. I have seen them clean a mess up off the floor then come to the counter and serve without washing hands.

One of the most unhealthy of the food myths is, 'more is better'.
Sugar
Not sure what you mean. NO food is unhealthy. What is unhealthy is over indulgence in any particular type of food. Over eat and you gain weight with any resultant health problems.
Unhealthy food have more sugar and its good, but healthy food dont have as much sugar and it tastes plain without sugar.
