The intake manifold insulator bolts are a critical failure item that has been identified with the Windstars.The symptoms of this problem are lean conditions bank 1 and 2, codes 171 and 174, along with rough idle.There is a terrific Photo-how-to-guide for this repair linked in the "Related Question" below.
That sounds pretty nasty... The secondary air system sounds pretty plugged up.I've assembled a number of related questions that could be related to this symptom.Check out the about a rough idle / codes 171 & 174 on a Windstar. This is about a TSB isssued by Ford and the Windstar Intake manifold "isolator bolts", AND a change in the valve cover design that causes lots of oil in the wrong places. Strikes me as something to be seriously looked into for your vehicle.

I'm not exactly sure about the 136 and 141 codes, but the 171 and 174 are symptomatic of a known problem with Windstar 3.8 litre engines.
