There were several outbreaks of plague in the empire of which the most famous was the Antonin Plague which occurred during Marcus Aurelius. While fighting in the east, Marcus' army picked up the plague and carried is back to the west. The terrible decease had a harrowing effect on the empire. There were lack of harvests, hunger was a common problem, there were to few men in the army which brought to severe barbarian invasions. Other plagues had similar effects.

The plagues could cause many deaths and thus disrupt production and trade.

The worse plague was towards the end of Justinian's rule. It was one of the worst in history. It has been called by historians the Plague of Justinian. Is impact on the empire has been compared to that of the Black Death of the Middle Ages. It probably affected the whole of Europe. It has been estimated that one quarter of the population died. Agricultural production declined. The populations of cities, which were crowded, was hit badly. The empire took a long time to recover.

The plague had nothing to do with the fall of Rome. Rome fell in 410 AD and the plague didn't show up unril the 1300's several hundred years after Rome.
