A chain of command is necessary to keep order and control. If there was no chain of command, workers would do as they wanted, with no one responsible for what went on. A chain of command clearly lays out the responsibility and authority of an organization. There should be no doubt in anyone's minds as to who they report to and who is in charge.
If the civilian is not in the officers chain of command the it is not possible to be insubordinate. If the civilian is in the chain of command then the process is exactly the same as insubordination to any other superior.

As long as they're not in the same command/unit (i.e., they're not part of the same chain of command), it's acceptable and also very common. What is strictly prohibited is fraternization between officers and enlisted. What I'm referring to as far as chain of command goes is that regardless of similar rank, one will always be senior to the other, and there's always the possibility of one being promoted over the other or being put in a position of authority over the other, and that's unacceptable.
Absolutely, as long as they're not within the same chain of command (i.e., the Chief isn't above the 2nd class in charge of her/him in the same command). Many Chiefs are in fact married to petty officers junior to them.
