There are many different ways you can tell if the rear axle has gone out on a Yamaha 350 ATV. If there is a grinding sound or a slipping when trying to put the vehicle in gear, those may be an indication of this issue.

There is no "standard" rear axle as most cutaway E-350s were used for ambulances, service trucks.... Look on the drivers door jamb for the axle code, then do a search for it (ford axle e3). My E-350 cutaway has an E3 axle with a 4.30 ratio.

The rear axle removal should be done only if safely and properly. The 1997 350 Wolverine should have an owner's manual that can provide information on how to do this removal. However, if the two nuts are missing, calling an auto shop or mechanic can provide more assistance.
There's only one axle...not a left and right. Buy a Honda service manual before you tare it up.
